this completed form to the State Youth Chair to be received by March 1. Rating year is from March 1 to
 February 28.  (Note:  For Clubs based on the school year, start with the beginning of the school year and
include any projects that will be completed by the end of the school year.)  Enter points for each category in
space provided.  Clubs earning 100 points will receive an Honor Roll Certificate; clubs earning 150 points
will receive a Special Achievement Award.  A club whose dues are not paid will not be eligible for a
Certificate or an Award.  Please list your projects on the back of this form.

 ALL YOUTH CLUBS
